Important Closing Information

Dear Valued Patient,

I hope this letter finds you in good health and spirits. I would like to take a moment to 
express my heartfelt appreciation for your loyalty and trust over the past 36 years. 
Serving you and your families has been an absolute privilege, and I am honored to have 
been a part of your healthcare journey.  However, the journey is not at an end as I look forward to continuing to care for you in a new location!

Effective Monday, October 16th, I be closing my Madison County Medical Associates 
office and will be joining the team at Health Trust Physicians Clinic (HTPC). This decision 
was made after careful consideration, and while change can be bittersweet, I believe that this move will bring about new opportunities for enhanced care and improved services for all 
of my valued patients. If you already have an appointment scheduled, the staff at the 
Health Trust Physicians Clinic will be reaching out to you to confirm the details of your 
upcoming visit at their clinic (300 W. Hutchings St, Winterset, IA).

After closing time on Friday, October 13th, you may reach me at the Health Trust 
Physicians Clinic by dialing (515) 462-2950. Rest assured the same level of 
compassionate care you have come to expect will continue throughout this time.

I also want to assure you that all your medical records from Madison County Medical Associates will seamlessly transfer to Madison County Memorial Hospital d/b/a Health 
Trust Physicians Clinic. Your health history is a crucial component of your ongoing care, 
and we are all committed to making this transition as smooth as possible, while 
safeguarding your privacy and confidentiality.

I have been working closely with the staff at Madison County Health Care System, and I 
am eagerly anticipating the start of this new journey.  I look forward to continuing to 
provide you with exceptional care under our unified practice.
